Having a problem with staying on task, huh? Who doesn't?
Believe it or not, all students (including students who get good grades) have to deal with bad procrastination habits. The key difference is that students who get good grades budget in time to procrastinate, whereas other students do not. Adding break times during your day helps to relieve stress, keep you energized, and make studying less agonizing. Instead of locking yourself in a room and planning to study for 8 hours straight, I would recommend that you study in 2-hour blocks and take breaks in between to stay fresh.
Moreover, there's research that shows that studying in large chunks of time is ineffective. Your mind absorbs information better in small chunks. So, keep that in mind as well.

I find that a lot of students run into procrastination issues, because they simply do not plan. When you get an assignment that's due in a week, don't wait until the day before the deadline to start. Instead, plan out how to complete small chunks of the project throughout the week, so that you complete the project little by little. This way, you put in 1-2 hours at a time over a few days, which is much easier to do than 10 hours in one sitting.
As my mother says, if you fail to plan, then plan to fail.
